2024 Judge Application Form

1General Information
This is the registration page for those who wish to judge the 2024 Canadian Pole and Aerial Championship (CPAC) Central Competition. Studio owners, instructors, and experienced competitors are welcome to apply to judge. Judging is a commitment to support the Canadian pole and aerial community.

As a CPAC Accredited Judge your mission is to create fair, unbiased, well-educated evaluations of pole and/or aerial performances in the CPAC competitions. The spirit of the Canadian pole and aerial community as well as the health and well-being of each athlete shall be your focus while acting as a CPAC Accredited Judge.

Judges are required to complete an online accreditation exam provided by the CPAC within two weeks prior to the event. As part of the online exam, we require our judges be transparent about any conflicts of interest that may present at the time of competition.

CPAC Accredited Judges will be matched by experience and skill to the offered divisions. Judges are charged with scoring and feedback of the performances of our competitors during the event on March 2-3, 2024. As part of the application below, please indicate the time commitment you are able to provide during the scoring period.

CPAC Judges will be featured on our social media, website and newsletter announcing who is judging. Following the conclusion of the 2024 CPAC you will receive an honorarium payment.
2Liability Waiver
I ACKNOWLEDGE AND AGREE that in participating in any activity hosted by Canadian Pole and Aerial Sports Inc. ("Canadian Pole and Aerial Championship", “CPAC”), whether in-person, off site, or in my own home, I am assuming any and all risks of personal injury, death or loss resulting from same.

Waiver of Liability
• I hereby waive, release, and discharge all claims that I have or may have in the future, and covenant not to sue CPAC, its administrators, directors, agents, officers, volunteers, employees, contractors, other participants, any sponsors, advertisers, owners, and lessors of the premises on which the activity takes place (each considered one of the "releasees" herein) from all liability, claims, demands, losses, damages, on my account caused or alleged to be caused in whole or in part by the negligence of the releasees or otherwise, including negligent rescue operations.
• I further agree that if, despite this release, waiver of liability, and assumption of risk, I or anyone on my behalf makes a claim against any of the releasees, I will indemnify, defend, and hold harmless each of the releasees from any loss, liability, damage, or cost, including attorneys' fees, which any of the aforementioned may incur as a result of such a claim.

Statement of Good Health
• I confirm that I am in good health and am capable of participating in the activities for which I am enrolling. If I have any concerns or uncertainty regarding my ability to participate, I will seek the advice of third-party medical professionals before participating.

Participation in Virtual Competition
• I understand that the independent nature of performing at my own location carries additional risk and will not hold CPAC responsible for injuries or damage that occurs as a result of my actions. I understand that I am solely responsibly for my own safety and the fitness of my equipment for use in any activity associated with this competition.

CPAC reserves the right to modify the terms of this waiver from time to time. I acknowledge that I have read and will abide by the latest rules and regulations, which can always be found on the CPAC website. (www.canadianpoleandaerialchampionship.ca)
Declaration
I hereby confirm that I have read this Liability Waiver, understand it and the implications of signing it, and declare that I am at least eighteen years of age and of sound mind and that I am signing this Liability Waiver voluntarily and of my own free will

Privacy Policy
CPAC must collect personal information in order to offer the services provided and will take all reasonable measures to ensure the safety and security of such data.

CPAC will never give out, sell, or otherwise make available the personal information of any client, except as required in the cooperation with legal proceedings. CPAC may use third party services, such as online streaming platforms, to facilitate our business services. By using these services, you are agreeing to the terms and conditions and privacy policies of these third-party sites as well.
