2024 Volunteer Application Form
1General Information
Join the Canadian Pole and Aerial Championship as one of our dedicated volunteers! CPAC events cannot run without the support of the Canadian pole and aerial community, and our awesome volunteers are what make an event a success!
This is the registration page for those who wish to volunteer for a 2024 Canadian Pole and Aerial Championship (CPAC) event. Please let us know when you can volunteer, and what roles you are comfortable in.
Backstage: Join the competitors backstage, guide them to being on time and ready for their performance. This role is perfect for moral supporters.
Pole Cleaner: See every performance and get your conditioning in at the same time! Clean and inspect the pole for every pole routine and assist with prop placement for all divisions. This is for the pole student already solid with their climbs, as you will be climbing 14 foot tall poles every 5-7 minutes! Pole cleaners should wear clothing that meets costume regulations.
Registration: Be a friendly, supportive face to everyone who attends our events. Check in athletes, check and sell tickets, and answer questions. This role is perfect for a social butterfly.

2Liability Waiver
I ACKNOWLEDGE AND AGREE that in participating in any activity hosted by Canadian Pole and Aerial Sports Inc. ("Canadian Pole and Aerial Championship", “CPAC”), whether in-person, off site, or in my own home, I am assuming any and all risks of personal injury, death or loss resulting from same.
Waiver of Liability
• I hereby waive, release, and discharge all claims that I have or may have in the future, and covenant not to sue CPAC, its administrators, directors, agents, officers, volunteers, employees, contractors, other participants, any sponsors, advertisers, owners, and lessors of the premises on which the activity takes place (each considered one of the "releasees" herein) from all liability, claims, demands, losses, damages, on my account caused or alleged to be caused in whole or in part by the negligence of the releasees or otherwise, including negligent rescue operations.
• I further agree that if, despite this release, waiver of liability, and assumption of risk, I or anyone on my behalf makes a claim against any of the releasees, I will indemnify, defend, and hold harmless each of the releasees from any loss, liability, damage, or cost, including attorneys' fees, which any of the aforementioned may incur as a result of such a claim.
Statement of Good Health
• I confirm that I am in good health and am capable of participating in the activities for which I am enrolling. If I have any concerns or uncertainty regarding my ability to participate, I will seek the advice of third-party medical professionals before participating.
Participation in Virtual Competition
• I understand that the independent nature of performing at my own location carries additional risk and will not hold CPAC responsible for injuries or damage that occurs as a result of my actions. I understand that I am solely responsibly for my own safety and the fitness of my equipment for use in any activity associated with this competition.
CPAC reserves the right to modify the terms of this waiver from time to time. I acknowledge that I have read and will abide by the latest rules and regulations, which can always be found on the CPAC website. (www.canadianpoleandaerialchampionship.ca)
Declaration
I hereby confirm that I have read this Liability Waiver, understand it and the implications of signing it, and declare that I am at least eighteen years of age and of sound mind and that I am signing this Liability Waiver voluntarily and of my own free will

CPAC must collect personal information in order to offer the services provided and will take all reasonable measures to ensure the safety and security of such data.
CPAC will never give out, sell, or otherwise make available the personal information of any client, except as required in the cooperation with legal proceedings. CPAC may use third party services, such as online streaming platforms, to facilitate our business services. By using these services, you are agreeing to the terms and conditions and privacy policies of these third-party sites as well.
